If you wish to create your album at home by selecting your composites on paper and filling in the album creation sheets by hand we will send you the information via email after we receive your album order, there is no fee for this.

Please select how you would like to create your album I would like to create my album online. The set up fee is $29.95 and must be paid before your album is prepared for online creation. This allows for 150+ layouts to choose from, including panoramics. I would like to make my album by filling the paperwork by hand. Limited layouts available. No panoramics available.
Album Delivery I would like to have my album shipped directly to me from the manufacturer for $9.95. Album will be shipped to the above address. I would like to pick my album up in the Belleville office once it is in. I understand that appointments must be arranged and are available Monday thru Thursday from 9am to 5pm.
This is the end of the Album Selection Process. If you have chosen any upgrades or shipping, please visit the Current Clients tab on our website to complete the payment for your options. Once we receive this order form and payment, 11-4 Productions will set your album up and email you further instructions of how to begin creating your album(s). Please remember that 11-4 Productions does not retain copies of finished products for more than 12 months. If your wedding was more than 12 months ago you will need to submit a non-returnable CD containing the images that you would like to use in your album.
