Payment Options (please choose one) * (1) Two Lump Sum Payments 1st due 180 days before wedding date and final payment due 60 days before wedding date. (2) I wish to pay everything in full at time of booking and receive $100 in free upgrades or add-ons. This excludes album upgrades and prints. Full Payment must be made within 7 days of booking to qualify. This offer may not be combined with any other offer. NOT ELIGIBLE IF WEDDING IS LESS THEN 180 DAYS AWAY.
*Payment Schedule: $400 retainer due at signing of this contract. $600 for reservation of 2 Photographers. One-half of the remaining balance is due 180 days before Photography/Videography begins. Remaining balance is due 60 days before the date of event and prior to receiving finished product. Payment can be made in the form of check, cash, money order or credit card. You can visit PayPal’s website at www.paypal.com. All payments through PayPal must be sent to sales@11-4productions.com. ALL CREDIT CARD AND PAYPAL PAYMENTS (EXCLUDING THE $400 DEPOSIT) WILL BE ACCESSED A 3% TRANSACTION FEE. NO EXCEPTIONS. Photos will be completed in approximately 8 weeks. The Small Print: 1. Payment Schedules and payment Info (a) If above stated payment schedule is not met, 11-4 Productions will not carry out the above services and any funds paid will be forfeited. (b) If the client is late on their first payment of 180 days before the event the entire balance is due before the event at the 90 day mark. (c) If client books within the 180 day mark, the deposit is due and the first payment is due 30 days after booking. (d) Delinquent accounts may be turned over to a third party collection agency or may be settled in small claims court for the state of Michigan. Any additional expenses incurred by 11-4 Productions in receiving final payment will be paid for by the client. (e) Final payment is due 60 days before your event. This does not mean your product will be finished at the 60 day mark. 11-4 Productions takes approximately 6-8 weeks for Photography editing and 3-4 months for Videography editing. (f) If client fails to pay at the 60 day mark before their event, 11-4 Productions will not attend said event and all funds paid are kept by 11-4 roductions. Late payments may delay delivery of your product in a timely manner. (g) Every 15 days delinquent a late fee of 5% of your current balance will apply. (h) 11-4 may change album options at anytime after booking due to suppliers. An equivalent album will be made available for all albums included in packages. (i) Due to high gas prices and 11-4 Productions continuing efforts to keep prices as low as possible. I understand that if my additional locations exceed 45 miles of traveling throughout the day from beginning location to final location a travel fee will apply. (j) Due to high gas prices and 11-4 Productions continuing efforts to keep prices as low as possible. I understand that if my wedding or reception is taking place more than 50 miles from our office in Orlando FL for a Florida event and more then 50 miles from our office in Plymouth MI for a Michigan event a travel fee will apply. To determine if I have a fee I understand I may simply use zip codes 48170 to measure distance, the rate is $0.55 per mile over the first 50 miles one way (or total of 100 miles round trip) per crew member. If you are having a destination wedding and have made prior arrangements with 11-4 those arrangements need to be outlined in the special request section of this contract. (k) Only one coupon may be used when booking or one deal. You may not combine special discounts and offers. * Read It
2. Editorial & Artistic Control (a) The client grants 11-4 Productions full artistic & editorial control of all aspects of production and post-production. (b) Personal photos and music selections for slideshows/signature mats/finished DVDs must be submitted within 30 days prior to your event or they will be excluded from your finished product. (c) 11-4 Productions will exercise reasonable care to insure the safe-keeping of any materials provided by client (such as photos); however, 11-4 Productions will not assume any liability for damages or loss. (d) All photos taken by 11-4 Productions are owned by 11-4 Productions; however, all photos may be duplicated by the below signed client at no cost as long as they are used for personal purposes and no monetary gain will be made from replication of said photos, nor may photos be published in any way without the consent of 11-4 Productions LLC. (e) All engagement shoots are done at a park chosen by 11-4 Productions, at a mutually agreed upon time for the client and 11-4. The client may choose a different park then the one made by 11-4 however a $125 fee will apply for a client chosen park. (f) 11-4 Productions reserves the right to use photos and audio/visual segments from the videotaping for promotional purposes. (g) 11-4 productions cannot guarantee the appearance of the subject(s) in the photographs. * Read It
3. Day of request, postponements, cancellations (a) In the event of postponement or cancellation of the event, 11-4 Productions reserves the right to keep funds paid. Packages may not be downgraded within 180 days of your event. Cancellation of your event within 180 days of your event will require complete payment of the agreed upon contract due 60 days before event was to take place. (b) Crews starting before 11am must be allotted a lunch break. 11-4 Productions earliest start time is 9AM unless it is pre-negotiated in the contract. (c) If working the reception of your wedding, a meal must be provided for the crew of 11-4 Productions. If we are doing the photography of your wedding, we request our own table be provided in or very near the reception area as we will be bringing both video and still cameras, their equipment, supplies, and a laptop with us and this helps to ensure the safety of this equipment that we need to capture your day in the best possible way. Please make sure to include us in the final head count for your reception site. The names on the place cards may read "11-4 Productions", regardless of who your 11-4 team is. Failure to do so will result in additional fees of $75 per person. We have found that seating us outside of the main room has resulted in missing events as we are not forewarned of them coming up or they are spontaneous. This ensures our crew will be in a better position to capture your day. If you have any questions concerning this please call the office for more explanation. (d) Crews will be assigned by 11-4 Productions. Requests for specific crews may be made but cannot be guaranteed. (e) Standard start time is no earlier than 9 am and standard end time at the reception is 10:45pm or once all standard main events have finished, whichever occurs first. Once we feel we are getting repetitive shots we will begin to wrap up the event. Any events occurring after 10:45pm must be pre-negotiated and written in the contract. If crews are needed to stay past 10:45pm a per hour fee of $200 for Photo Only or Video only or $400 for Photo/Video combo will apply (unless an hourly package has been purchased). Additional requests are charged accordingly at a per hour rate. (f) 11-4 Productions will attempt to comply with the requests of the client and this contract; however, no guarantee is expressed or implied. (g) In the event of equipment failure, videotape failure, personal injury or illness, or errors of any kind, wherein 11-4 Productions fails to comply with this contract, 11-4 Productions’ liability is limited to a refund of funds paid. (h) If you downgrade from a Photo/Video combo package to a video only package a $200 fee will apply to your new package. (i) It is 11-4’s policy that our crew members do not ride in the limo along with the bride and groom or bridal party. 11-4’s crew will simply follow behind in their own vehicle. * Read It

I understand that other people may select the exact same photographers I have selected multiple times and we at 11-4 Productions can not duplicate ourselves and be everywhere at once. I understand that 11-4 Productions in no way guarantees or implies I will receive one of my top choices. I understand that for the 1/4 Carat Package no request for photographers are taken.
